The Alarming Reality of Medical Scan Errors at HSE Hospitals

Mistakes in reading medical scans have far-reaching consequences. From 2020 to 2024, a staggering 193 errors in medical imaging were reported in HSE hospitals, according to new data. While some errors were minor, 24 were classified as major or extreme, leading to severe harm, disability, or even death. These revelations are a sobering reminder of the critical role accuracy plays in medical care and the devastating impact of lapses in diagnostic reporting.

Understanding the Scale and Impact of Medical Scan Errors

The figures from the Health Service Executive (HSE) paint a concerning picture. Of the 193 reported cases:

  • 24 were classed as major or extreme – these include incidents resulting in death, permanent disability, or severe harm to bodily, sensory, or intellectual functions.
  • 67 were moderate in severity, impacting patients’ health and hospital stays.
  • 102 were considered minor, though any error in medical diagnostics has the potential for profound consequences.

Major or extreme incidents go far beyond statistical data. They often involve outcomes such as brain damage, loss of limb, or blindness, leaving patients to grapple with life-altering complications. Others may face extended hospital stays or significant alterations to their daily lives, all stemming from avoidable errors.

Aontú Senator Sarah O’Reilly has voiced patient concerns, highlighting how these errors deeply undermine public trust in healthcare.

“The idea that so many scans are being misread or misreported in our hospitals is a source of worry for patients,”

she stated, pointing to system-wide deficiencies, such as outdated diagnostic equipment, that contribute to these failings.

The Need for Accountability and Transparency

The HSE reports acknowledge the gravity of these errors, noting that 34 cases warranted further review, with 15 cases resulting in specific recommendations to improve patient safety. However, one critical question remains unresolved—have all affected patients been informed of these errors?

Ireland’s healthcare system has faced scrutiny over its handling of patient communication in the past. The cervical check scandal, which unfolded over several years, underscored the need for open disclosure when errors occur. Senator O’Reilly plans to press for clarity on this issue in the Seanad, particularly around whether all victims of diagnostic errors have been notified.

she stated emphatically.

The HSE’s Response and Its Limitations

The HSE has sought to address discrepancies in radiology through the National Radiology Quality Improvement (NRQI) Programme. This initiative aims to enhance system-wide radiology practices by focusing on improvement through feedback, education, and departmental peer reviews. However, the programme does not intervene in individual cases or gather personal data—an omission that leaves gaps in accountability and patient assurance.

Majella Daly, assistant national director at the National Centre for Clinical Audit, has pointed out that radiology errors, while uncomfortably common, do not necessarily equate to negligence. Factors such as human error and system inefficiencies inevitably contribute to discrepancies in radiological reporting. However, while some degree of error may be unavoidable, patients deserve a system that mitigates risks through robust safeguards.

One glaring concern is the outdated technology in some HSE hospitals. Timely diagnostics often depend on state-of-the-art equipment—a plea echoed by medical professionals and policymakers alike. When lives are at stake, investing in modern tools and training is non-negotiable.

Furthermore, while the NRQI initiative has laid a foundation for improving radiology practices, its framework prioritises systemic education over addressing the needs and rights of individual patients directly harmed by these errors.
